​Long Term Acute Care Hospital (LTACH)​​

The Long Term Acute Care Hospital is a dedicated inpatient medical rehabilitation program providing continuous and intensive medical, nursing, and rehabilitative care to medical complex patients whose severity of illness or injury requires a chronic level of care.  Patients require frequent physician interventions, nursing assessments, and ongoing coordination rehabilitation services to achieve their maximum level of independence and discharge to home or the least restrictive environment if home is not possible.

This high level of care includes patients requiring life support such as ventilator management and weaning, and enteral and intravenous nutrition. The program also serves individuals with significant functional impairments related to injury or illness who require intense physical rehabilitation.

Services are provided through a comprehensive, interdisciplinary team approach, in which all person’s programs are tailored to meet their individual needs. We strive to provide patient centered care, adapting our structure to accommodate different cultures, age, gender, sexual orientation, spiritual beliefs, socioeconomic status, language, and personal preferences.  Our program holds the seal of approval from the Commission on Accreditation of Rehabilitation Facilities (CARF), an independent, nonprofit accreditor of health and human services. This means CARF recognizes our team for their skills and commitment to your safety.​​
